Our Care Model

Medical needs, mental health needs, and social needs all affect each other. In the Imagine Pediatrics Care Model, our pediatrician-led team creates care plans alongside caregivers and providers based on the child’s medical, mental health, and social needs. When all types of care work together, outcomes improve and children and youth with special health care needs can have more Safe Days at Home.

Creating More Safe Days at Home

Safe Days at Home are what everyone wants for children and youth with special health care needs. These are days when children are stable and doing well at home, and parents and caregivers feel confident and supported in managing care.

For health plans, more Safe Days at Home means less avoidable hospital visits and outcomes. For providers, it means patients get ongoing support between appointments. And for families, it means more comfortable days at home and in the community making memories.

Supporting Children and Youth with Special Health Care Needs

One in four children needs extra health support – that’s over 14 million families.

Children and youth with special health care needs live with ongoing medical, mental health, or developmental conditions that require extra support. This includes children with conditions like autism, ADHD, diabetes, cerebral palsy, rare genetic disorders, and many others. These families often navigate multiple doctors, therapists, medications, and appointments while managing everyday life. Our care model is built specifically for these children and families, providing coordinated medical, behavioral, and social support through one dedicated team that knows each child’s unique needs.
