Who We Are
Imagine Pediatrics is a tech-enabled medical group that delivers 24/7 integrated virtual and in-home pediatric complex care for children with special health care needs. Our multidisciplinary team brings together board-certified pediatricians, complex care doctors, advanced practice providers, nurses, behavioral health therapists, pharmacists, dietitians and social workers into one team to deliver integrated medical, behavioral and social support.
Imagine Pediatrics offers a proven, scalable value-based pediatric care model with a local community presence. Through partnerships with risk-bearing organizations through total-cost-of-care and quality performance-based risk arrangements, and alongside the child’s existing providers, specialists, and health plan care management teams, Imagine Pediatrics expands access to personalized care that improves outcomes, experiences, and costs of care so kids can spend less unnecessary time in the hospital and have more Safe Days at Home.
