Every child’s health journey looks different
Children with special health care needs have experiences that can look very different from one child to another. A child’s needs may look different depending on their medical condition, age and stage of development, mental health, home and family, and access to healthcare and community support. Most children with special health care needs have more than one condition and need care across several areas at a time. Their needs often fall into three categories: medical care, mental health care, and social support.
-
Medical Care
Some children live with complex medical conditions that need ongoing treatment and monitoring. They might need special medical equipment like feeding tubes or oxygen support. Children with special health care needs often need help managing their medications and coordinating care across specialists.
-
Mental Health Care
Many children with special health care needs have mental health conditions that affect how they learn, communicate, and interact with themselves and others. Mental health support includes behavioral health care, counseling or therapy, medication support, and strategies to help children succeed at home and in school.
-
Social Support
A child’s health is shaped by more than medical care alone. Many families also need support with everyday challenges that can affect a child’s health and wellbeing. Social support helps with access transportation, food, and housing. It also coordinates care between doctors, schools and community services while connecting families with local support programs. Social support helps caregivers manage the day-to-day needs of caring for a child with special health care needs.

How Imagine Pediatrics Helps Families Have More Safe Days at Home
Imagine Pediatrics provides 24/7 medical, mental health, and social care that supports families before problems become emergencies. Our care model is purpose-built for children with special health care needs connecting every part of your child’s care.
-
A Dedicated Care Team That Knows Your Child
Your child does not get a different care team every time they need help. They have one dedicated team the same pediatricians, nurses, therapists, pharmacists, dietitians, and care partners – who get to know your child’s history, conditions, medications, and what matters most to your family. When your care team already knows your child, they can spot changes faster, respond more confidently, and make better decisions alongside you.
-
Proactive Care That Prevents Emergencies
Our care teams focus on getting ahead of problems before they become emergencies. By providing support around the clock and staying connected between appointments, we help reduce unnecessary emergency department visits and hospital stays so your child can spend more time at home, at school, and with family.
-
Virtual and In-Home Care, Day or Night
When something feels off, you do not have to wait until morning or drive to an urgent care center. Open the Imagine Pediatrics Mobile App and message your care team the same team that already knows your child. Within moments you can be on a video call with a pediatric nurse or doctor. In select areas, our Mobile Integrated Care team can come to your home for sick visit assessments, rapid testing, labs, and more, all without your child leaving home.

Kelli’s Story
Imagine Pediatrics showed up in a time of crisis, and for the first time in their lives, I didn’t have to take my boys to the ER. Any time something comes up with my boys, either medically or psychiatrically, Imagine Pediatrics is now my first call.
Kelli, mother and caregiver
This partnership with Imagine Pediatrics is so unique and special and it makes such a big difference in the lives of our members. When you look at Kelli’s story, it’s so impactful because you realize the difference we can make in the lives of our members and their families when we come together for the common good.

The Challenge — Grouped Sample
14.5 million Children with Special Health Care Needs (CSHCN) in the U.S. account for 50% of pediatric spend and are currently served by a fragmented, uncoordinated, and inaccessible system — leading to high utilization and suboptimal outcomes.
Barriers to Effective Care — Grouped Sample
-
Fragmentation of Care and Data
Children with special health care needs rely on multiple providers, systems, and programs — yet these touchpoints rarely communicate in ways that support continuity.
-
Policy Barriers
Variation in Medicaid policies, reimbursement rules, and program eligibility creates access gaps, disrupts continuity, and makes it difficult for families to navigate care consistently over time.
-
Caregiver Fatigue and Burnout
Families often shoulder the responsibility of care coordination, provider communication, school advocacy, and crisis management — all while balancing jobs, other children, and daily life.
-
Unique Needs of Each Child and Family
No two children with special health care needs are alike. Medical, behavioral, and social needs evolve quickly, requiring highly personalized, flexible support that most systems are not built to deliver.
